Questions Families Ask about Care

What Types of Care Does Crowned Legacy Care Provide?

Crowned Legacy Care provides Companion/Sitter Services, Personal Care Services, and Nursing Services. Services are based on the individual needs of the client and the services included in the client’s service agreement and service plan.

Companion/Sitter Services may include companionship, emotional support, games and activities, transportation and escort, light meal preparation, light housekeeping, walks, and medication reminders. Companion/Sitter Services do not include personal or hands on care.

Personal Care Services may include assistance with bathing, grooming, dressing, eating, toileting, transfers, positioning,  ambulation, oral hygiene, and other authorized activities of daily living. Services are provided according to the client’s individualized service plan.

Our Nursing Services may include nursing assessment and observation, vital signs, chronic disease management education, medication administration and management according to written physician orders, wound care according to physician orders, and care plan management and evaluation.

Our Nursing Services are available to individuals who need nursing support in their homes and communities, including elderly individuals, individuals with disabilities, post operative clients, and workers compensation clients, when the requested services are within our scope.

Each service plan is individualized around the client’s needs, preferences, functional limitations, required services, frequency and duration of care, goals, and discharge needs. The client and responsible party participate in planning when applicable. When nursing services are provided, the client’s physician is involved as required.

Services are established through written service agreement that identifies the services to be provided, expected frequency and duration, and applicable charges. Services are then delivered according to the client’s individualized service plan.

Contact Crowned Legacy Care to discuss your loved one’s needs and the type of support you are seeking. A supervisor will help determine whether the requested services are within our scope and, when appropriate, develop an individualized service plan and service agreement.
